1. When taking your shift report, whether verbally or by listening to a tape, this is the once chance you'll have to learn what you'll need to know about your patients before you take responsibility for them.

  • In a hospital, (we'll be adding a nursing homes section soon) be sure to ask the nurse you're getting report from before you start (or note them as you go), are there any:

  • Pre-op patients that are going to surgery (the Operating Room) that shift (you'll need to make sure a pre-op checklist has been completed). You might want to call the OR an find out what time they are on the schedule as well. Major delays due to unfinished paperwork can frustrate both the patient, and the operating room staff, transport personnel.

  • Any patients in Restraints, if yes why? Hospitals and Nursing homes have strict requirements as far as documentation for patients in restraints. Make sure a restraint documenttion form has been completed for the prior shift and that you7 have one for you current shift. The wise nurse would double check he chart to be sure that there is an order for the restraints as well. Most expire within 24 hours and need to be renewed daily according to the hospital or facility policy.

  • Any patients going for tests or procedures. If a patient's going for a Esophagoduodenoscopy (EGD), or a Colonoscopy, they'll likely need a prep. Commonly GI patients will need Enemas, Ducolax or Go Lytely, to clean out their systems prior to the test. A patient going for a CT Scan of the Abdomen or Pelvis may need to drink oral cntrast prior to the test.

  • Any patients with Laboratory testing to be done or to come back (you'll be responsible for checking the labs and calling in any critical values to the patient's physician).
