Hospice being marketed as a cost-cutter for hospitals, USAToday, #nurseup #nursefriendly #hospice #eolchat:"By Kelly Kennedy, USA TODAY:"Hospice marketers, exploring possibilities for new revenue to help continue the industry's remarkable growth, are looking to exploit a provision in the 2010 health care law by persuading hospitals to send Medicare patients into end-of-life hospice care instead of readmitting them to the hospital.
Such a move, the hospice marketers say, will enable hospitals to avoid paying the Medicare penalties required by the new law when hospitals discharge patients and then have to readmit them within 30 days: Instead of readmitting the patients, hospitals should send them to hospice care, which also is covered by Medicare, according to a USA TODAY analysis of marketing materials.
Patients with severe heart problems and pneumonia tend to decline quickly and often move in and out of hospitals, said hospice marketing specialist Rich Chesney, who proposed the idea."

Surgery leaves incisional pain as well as internal hurts from the removal of a gallbladder or tumor, or the repair of a broken bone. Chemotherapy weakens the immune system. We might like to say it shouldn't be, but physical pain, and its concomitant emotional suffering, tend to be inseparable from standard care.
What's more, recent research suggests that judging care in terms of desirable customer experiences could be expensive and may even be dangerous. A new paper by Joshua Fenton, an assistant professor at the University of California, Davis, and colleagues found that higher satisfaction scores correlated with greater use of hospital services (driving up costs), but also with increased mortality."

Florida hospitals stuck with long-term, costly ventilator care - St. Petersburg Times:" Larry Brazil was a patient at University Community Hospital for almost three years. Tameka Campbell was at Tampa General Hospital for five years.
Together, they had bills of nearly $11 million that no private or public insurance plan will cover. That pricey care did not cure them; it kept them breathing on ventilator machines.
Brazil and Campbell could have been cared for at a fraction of the cost — if Florida had the right kind of nursing facilities.
State records indicate that fewer than two dozen nursing homes — including three in the Tampa Bay area — out of about 700 statewide care for ventilator patients. But some only care for children or patients who can likely be weaned off the machines. Only a handful take new patients, and their ranks are dwindling."

Progressive Care Unit, Cardiovascular and Pulmonary Services, Mercy Medical Center:"Mercy's Progressive Care Unit is a 20-bed cardiac telemetry monitoring unit for adult patients with a variety of acute and chronic illnesses. Patients with cardiac, medical, surgical, neurological, and post-trauma diagnoses are accepted.
The interdisciplinary care team consists of specially trained registered nurses, nutritional services, physical therapy, cardiac/pulmonary rehabilitation and counseling services. An outcomes manager works closely with patients who have chronic cardiac conditions and their families to provide education and support.
The specially trained staff oversees centralized cardiac monitoring of patients located on the Orthopedic, Surgery, Neurology/Oncology and Medical Units as well as Progressive Care."

Spotlight on Surgical Progressive Care Unit (SPCU), Sue Thompson, BSN,RN,BC, Southeast Missouri Hospital:"The Surgical Progressive Care Unit (SPCU) specializes in pre and post-operative patient care. Cardiac monitoring, administration of anti-arrhythmic medications, and post-operative open-heart, vascular, and thoracic surgery care comprise the most frequent diagnoses on SPCU. In addition, chest tubes, temporary pacemakers, and telemetry monitoring of patients on Oncology and Neurology is routine for this unit.
A 15-bed patient care unit, SPCU is managed by Evelyn Mills, RN,C, and assistant manager Becky Tripp, BSN, RN,C. Staff includes 15 RNs, five perdiem RNs, one LPN, nine nursing assistants, and five unit secretaries. The mission of SPCU is to provide optimal patient care while meeting the needs of the patient's family. Evelyn states, "I am honored to be part of a team that has so much passion for nursing, and this is reflected in the care patients receive."
